Our Health Information Management team is here to help patients and caregivers easily access medical records through the Patient Portal. If you are a patient and have not yet set up portal access, you can contact our office and provide an email address to receive an invitation to get started.
Once enrolled, the Patient Portal allows you to:
- Review past appointments
- Communicate with your provider
- View test results
- Share your records with other healthcare providers
If you are unable to use the Patient Portal and need a copy of your medical records, you can complete a Medical Records Release Authorization form and return it to our office for processing.
